Clinic Information 

Welcome to the office of John Forsyth, Registered Massage Therapy. To ensure a smooth first visit, please review our clinic policies and download the necessary forms below.

Clinic Policies & Safety

  • Health & Wellness: We are a healthcare facility practicing communicable disease prevention. If you are feeling ill or have a contagious skin condition, please call to reschedule.
  • Scent-Free Zone: To accommodate sensitivities, we are a strictly scent-free environment. Please refrain from wearing perfumes, colognes, or emitting strong scents from oils, chemicals, or smoke.
  • Masking: Masks are available on-site for your convenience.
  • Professional Standards: Per regulatory requirements, the application of essential oils or cannabis derivatives is not permitted.

Oils & Allergy Information

We use high-quality, hypoallergenic lubricants. Please notify your therapist if you have specific nut or oil sensitivities?
  • Holly Tree Oil: Hypoallergenic.
  • Available Carriers: Apricot Kernel, Avocado, and Coconut (MCT) oils.
  • Biotone Revitalizing: Contains Sweet Almond oil, Coconut oil ester, Hawaiian Kukui nut oil, Vitamin E, and Olive oil.

Massage Techniques

Our treatments incorporate a variety of evidence-based modalities, including:

  • Systemic Deep Tissue Therapy (SDTT) & Trigger Point Therapy (TRPT)
  • Soft-Tissue Release (STRT) & Myofascial/Connective Tissue techniques
  • Dermoneuromodulation (DNM) for peripheral/superficial nerves
  • Orthopaedic joint mobilizations and Dr. Vodder’s Basic Lymphatic drainage
  • Static pressure and friction techniques.

Your First Appointment

  • Arrival: Please arrive 5–10 minutes early to complete necessary paperwork.
  • Duration: A 60-minute subsequent appointment includes the full cycle of care: administration, assessment, treatment, and home-care education.
  • Intake Forms: A Patient Intake Form (PIF) is mandatory. You may download it below to fill out ahead of time, but it must be signed in the presence of the RMT.

    • Privacy Note: Your information is confidential and protected under BC PIPA.
    • Language Barriers: If needed, an interpreter may assist with the form and be present during treatment.

General Patient Files:

Insurance Benefit Consent Forms:

If you require direct billing, please complete the form corresponding to your provider: 
  • Pacific Blue Cross Consent Form
  • Telus eClaims Consent Form (For all insurers under the Telus portal)
  • SunLife Member Consent Form
  • Green Shield (GSC) ProviderConnect Form

Fees & Receipts

  • Payment Policy: Cash, cheque or e-transfer only. To keep our rates competitive and local fees low, we do not accept credit or debit cards at this time.
  • Receipts: Standardized receipts are issued upon request once the patient's portion is paid in full. And typical made available within 1-2 business days. If insurance (including ICBC Direct-bill) covers 100% of the service, a receipt is not issued unless it is a "member paid" service where the insurance pays the member directly, and the patient (member) then pays the clinic (provider) such notation is made on receipt.

Patient Intake Form - must be filled out for your initial appointment.

This is a mandatory patient intake form * (PIF) that must be completed prior to initiated therapist-patient relationship and or prior to treatment to commencing. This is done at the first appointment.

* It's information is protected by BC PIPA and confidential!

Insurance forms:  Insurance consent forms organized by Direct-billing Portal:

  • "consent" form is mandatory condition for direct-billing

​If request by Clinic, any patient (or patient to be) of this clinic may download this file, fill it out (except the "Provider" section and sign at clinic) and email or bring to (first appointment) the Clinic

  • ICBC has its own portal.

  • Pacific Blue Cross, SunLife and Green Shields Connect (products) have their own portals.
 
  • Telus portal provides access to all other Insurers (see Insurance page)

Receipts info: 

Receipts* are issued by request, only when a patient portion has been paid in-full.

Insurance paid service portion will be noted on a receipt.  Services (100%) fully paid by insurance will not get a receipt 
with exception in the case of "member paid" service and member has to pay provider (RMT)

* Receipts are standardized by our regulatory college to this format: "receipts.pdf".

​Note: this Clinc's policy on fees is to keep them as low as possible, so we have reduce the costs of credit-debit services by not having those methods of payment. And from time-to-time, due inflation of running a business results in marginal increases in prices. And for the local area price we are not the highest.
